It looks like your submittal template is out of date. The last version includes the “Title” input in that bold text that says Submittal. Here’s how to update your Template: Updating Templates
You may also be interested in this relatively new feature as well that lets you customize the subject line of your notification emails.
If either of those are not enough we have another request on the board for customizing the body of the email notification that you’re welcome to upvote here.

Thank you. I was able to update it successfully. I don’t have any forms to close at this minute, but I will check back in on this when I do!
